ALL FRESHMEN AND TRANSFEREES WHO PLAN TO ENROLL FOR THE FIRST SEMESTER OF THE ACADEMIC YEAR 2023-2024 MAY NOW REGISTER THROUGH ONLINE e-ADMISSION (LINKS BELOW). Only students with complete requirements will be admitted.

FRESHMEN
1. The original and three (3) photocopies of your Grade 12/Senior High School card (F138) duly signed by the head of your school with a rubber-stamped facsimile should be countersigned by the principal or his/her authorized representative.
2. The original and three (3) photocopies of your Certificate of Live Birth (PSA Certified).
3. Good Moral Certificate.
4. Two (2) recently colored 2” x 2” pictures with a printed name tag and the signature of the student at the back.
FOR TRANSFEREES:
1. Transcript of Records (TOR).
2. The original and three (3) photocopies of Honorable Dismissal/Transfer Credentials.
3. The original and three (3) photocopies of your Certificate of Live Birth (PSA Certified).
4. Good Moral Certificate.
5. Two (2) recently colored 2” x 2” pictures with the printed name tag and signature of the student at the back.
